Dear Valued Patients and Families:

The providers and medical team at Issaquah Medical Group have been delighted to serve you and our community since our opening in 2000. However, after 11 years, IMG will be making some changes in October 2011.

Dr. Tadd Hsie will be moving to UW Physicians in Issaquah on October 1st, 2011.

Dr. David Ege will be moving to Swedish Medical Center in Issaquah on November 1st, 2011.

Dr. Sandra Sjoberg will be moving to Pediatric Associates in Factoria on November 1st, 2011.

Please note their new contact information. If you would like your medical records transferred to the new offices for Dr. Hsie, Dr. Ege, or Dr. Sjoberg, please complete a transfer of records form.

Dr. Elizabeth Lehmann-Taylor, Dr. John Henn and ARNP Keya Chisholm will be remaining in the same office in Issaquah under new management of Overlake Medical Clinics. Their patients will not need to transfer their medical records or contact information.

Please understand that you may receive statements from IMG after November 1st, 2011, as insurance processing must occur before billing statements are delivered. These statements will be for services provided prior to November 1.
